Understanding pain with an ACL tear
The anterior cruciate ligament helps control forward movement and rotation of the knee. A tear may cause pain, swelling, a pop at the time of injury, reduced movement, and a sense of instability. Pain can also come from other injured structures in the knee, so its location and timing matter.
Pain that gradually settles can still need follow-up, especially when the knee feels unreliable during turning, stairs, or uneven ground. Examination helps assess stability and whether additional structures may be involved.
What you can do
After a new injury, stop pivoting, running, and jumping. Rest from painful loading, elevate the leg when comfortable, and use a wrapped cold pack for short intervals. Use support or crutches only as advised, and do not test the knee by repeatedly twisting it.
Arrange orthopaedic or physiotherapy review to discuss movement, strength, and a safe return to activity. Note whether swelling appears quickly, what triggers pain, and whether the knee gives way.
When care is urgent
Get urgent assessment if the knee is visibly deformed, becomes rapidly swollen, is locked, or cannot take weight. Immediate help is needed for a cold or pale foot, new numbness, or severe weakness below the knee.
Contact your treating team sooner if pain is escalating, sleep is repeatedly disrupted, the knee repeatedly buckles, or you develop fever with increasing redness and warmth. These changes need examination rather than self-management alone.
