When to Worry About Knee Clicking With Plica Syndrome

Short answer
Knee Clicking and Grinding is a common presentation of Plica Syndrome. Arrange care when the sound becomes painful, persistent, or linked with swelling, catching, locking, or reduced movement.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common trigger
- Repeated bending, stairs, or a training increase
- Seek prompt care
- Locking, sudden swelling, or inability to bear weight
Understanding plica-related clicking
A plica is a fold of joint lining that can become irritated and thickened. It may rub across nearby structures as the knee bends, producing a snap or click around the inner side of the kneecap. Repetitive bending, running, stairs, or a recent increase in training can aggravate it. Pain with pressure beside the kneecap and tenderness are more informative than an isolated sound.
Ways to settle irritation
Temporarily reduce repeated stairs, kneeling, deep bends, and running if they reproduce symptoms. Keep gentle range-of-motion movements within comfort and return to loading gradually. A physiotherapist can examine hip and thigh control, flexibility, kneecap movement, and training habits, then prescribe strengthening. Avoid pressing directly on a painful area or trying to snap the knee into place.
When to contact a clinician
Seek a routine assessment if symptoms do not settle, interfere with work or sleep, or keep returning after activity changes. Get prompt care for sudden swelling, inability to straighten the knee, inability to bear weight, a major injury, or a hot red joint with fever. Those features may need evaluation for another or additional knee problem.
Questions to bring
Ask where the plica is likely irritated, which movements to modify, and how to restart sport or exercise. Ask what findings would make imaging useful and how you should respond if locking, swelling, or pain suddenly increases.

Questions people ask
Can plica syndrome improve with physiotherapy?
Many people are managed with activity modification and targeted rehabilitation, guided by symptoms and examination findings.
Is a knee click alone a sign of serious damage?
A click alone is not enough to judge severity; pain, swelling, locking, injury, and loss of function are more meaningful warning signs.
