When to worry about itching in pregnancy with obstetric cholestasis

Obstetric cholestasis affects how bile is handled during pregnancy and can cause itching, often without a visible rash. It may be strongest on the palms or soles and can spread. Pregnancy-related itching has other possible explanations, so symptoms need assessment rather than self-diagnosis.

Tell your midwife, obstetrician, or maternity unit about new or worsening itching, even if your skin looks normal. Mention where it is, when it began, whether it interrupts sleep, and any dark urine, pale stools, yellowing skin, or reduced fetal movement. Your team may arrange blood tests and follow-up based on your symptoms and results.

Contact your maternity service urgently if itching is intense or rapidly worsening, or if you notice yellowing, dark urine, pale stools, abdominal pain, or reduced fetal movement. Reduced movement needs immediate maternity guidance. Do not wait for a routine appointment when you are concerned about your baby's movements.
