When to worry about inability to bear weight with a tibial plateau fracture

Short answer

Inability to bear weight is a common presentation of tibial plateau fracture, but worsening pain, a cold or numb foot, marked swelling, or new colour change needs prompt medical assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Protect the knee
Use the prescribed support and weight-bearing restriction.
Check the foot
New coldness, numbness, weakness, or colour change needs urgent assessment.
After a setback
A new fall or sudden worsening warrants prompt contact with the treating team.

What inability to bear weight can mean

The tibial plateau is the upper surface of the shinbone that helps form the knee joint. A fracture there can make standing painful and may affect the joint surface or the stability of the knee. Your orthopaedic team may therefore ask you to avoid weight on the leg while the bone heals or until an operation and follow-up assessment establish a safe progression.

Not being able to walk immediately after the injury can fit the fracture. Concern rises when the limitation is new after a period of improvement, follows another twist or fall, or comes with pain that is difficult to control. A sudden increase in tight swelling can also signal a problem that should not wait for a routine appointment.

What to do now

Follow the prescribed weight-bearing limit; do not test the leg by standing without the recommended brace, crutches, or other support. Keep the knee positioned as instructed, elevate the leg when resting if your clinician has advised it, and use pain treatment only as directed. Check the toes for warmth, sensation, movement, and colour, and note whether swelling or pain is changing.

If you are unsure whether a step or amount of weight is allowed, contact the treating orthopaedic team rather than guessing. A physiotherapist can show you safer transfers, crutch technique, and exercises that protect the knee within your current restriction.

When to seek care

Seek urgent care for a foot that becomes cold, pale, blue, or newly numb; severe or rapidly increasing pain; swelling that feels unusually firm or tight; inability to move the toes; or a splint or brace that feels too tight. Get prompt medical advice after a new fall, a sudden change in alignment, wound drainage, fever, or pain that is escalating despite the agreed plan.

Call emergency services if severe leg symptoms occur with chest pain, sudden breathlessness, fainting, or coughing blood. If the leg remains unable to bear weight when your clinician expected gradual progression, arrange review so the fracture, brace, and rehabilitation plan can be checked.

Questions for your doctor

Ask: What is my exact weight-bearing limit, and when will it be reviewed? Which brace position and exercises are suitable now? What changes in pain, swelling, toe sensation, or skin colour should trigger an urgent call? When should I expect imaging or a follow-up examination to guide the next stage?

Questions people ask

Is inability to bear weight expected with a tibial plateau fracture?

It can be expected because the injury affects a weight-bearing part of the knee. The permitted amount of weight depends on the fracture pattern, treatment, stability, and your clinician’s examination.

Should I walk on the leg to see if it is improving?

No. Use only the weight and support authorised by your treating team. Testing the leg can worsen pain or place stress on healing bone.

What foot changes are especially concerning?

New numbness, weakness, coldness, pale or blue colour, or difficulty moving the toes should be assessed urgently, particularly when swelling or pain is also increasing.
