When to worry about heel pain with a stress fracture

Short answer
Heel Pain is a common presentation of Stress Fracture, and pain that worsens with loading should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Concerning pattern
- Pain that increases with loading or continues after activity.
- Until assessed
- Avoid impact exercise and repeated self-testing of the heel.
What the pattern can tell you
A stress fracture is a small bone injury that can develop when repeated loading exceeds the bone's ability to recover. Heel pain may begin during activity and later linger afterward. A precise tender spot, pain with ordinary walking, swelling, or a recent increase in training makes an examination especially useful. Symptoms can overlap with soft-tissue conditions, so the location and behaviour of pain matter.
Protect the heel
Stop running, jumping, and other impact activity until you have received clinical advice. Limit walking to what is necessary, use supportive footwear, and do not test the heel repeatedly to see whether it still hurts. Note when the pain began, any training or work change, and whether rest reduces it. A clinician can decide whether imaging or protected weight-bearing is needed.
When to seek care
Arrange prompt assessment for pain that persists, increases with each step, or returns quickly when activity resumes. Seek urgent care if you cannot bear weight, the heel becomes significantly swollen or bruised, pain follows a major injury, or the foot becomes numb, cold, pale, or weak. Fever or spreading redness also needs urgent attention.
Questions for your doctor
Ask whether the pain location suggests a bone injury, what weight-bearing is appropriate, whether imaging is needed, and how to return to work, training, or sport.

Questions people ask
Can I walk on a painful heel?
Walk only as much as necessary until advised, especially when each step increases pain or causes a limp.
Does less pain rule out a bone injury?
No. Symptoms can fluctuate, so persistent or load-related pain still merits clinical assessment.
