When to worry about heavy bleeding after birth with retained placenta

A retained placenta means placental tissue remains in the uterus after birth. The uterus may not tighten effectively while that tissue remains, which can lead to ongoing bleeding or bleeding that returns after it had eased. Cramping, pelvic pain, fever, unpleasant-smelling discharge or feeling weak may also occur. These features overlap with other postpartum problems, so examination is needed.

Contact your maternity unit urgently if bleeding becomes heavy, returns after slowing, or is accompanied by pain, fever or dizziness. Keep track of the flow and clots, rest with support nearby, and bring your postnatal notes if attending care. Do not wait for a scheduled check when the pattern is worsening.

Call emergency services for fainting, collapse, confusion, severe weakness, breathlessness, chest pain, a pounding heartbeat, or bleeding that rapidly increases. Fever with pelvic pain or offensive discharge needs same-day review. Clinicians may need to assess the uterus and blood loss quickly.

Ask whether retained tissue is suspected, which examination or imaging is needed, and what treatment options are available. Ask how to monitor bleeding and when infection symptoms should prompt an immediate return.
