When to Worry About Hand Swelling With a Distal Radius Fracture

A distal radius fracture can irritate injured tissues around the wrist and hand. Fluid may move into the fingers, especially when the arm is kept down. A splint, cast, or dressing can then feel tighter as swelling changes. Finger stiffness is also common when movement is limited.

Swelling should be considered alongside sensation, finger movement, skin colour, and pain. A gradual change may be part of healing, while a sudden change after a new injury, tighter support, or increased pain deserves review.

Keep the injured arm elevated above heart level when resting, and move the fingers gently if your fracture team has permitted this. Follow the cast or splint instructions, keep the support dry, and remove rings before swelling increases. Do not loosen, cut, or adjust a cast yourself unless a clinician instructs you.

Check the fingertips for normal warmth, colour, and feeling. Arrange follow-up if swelling remains troublesome, the support rubs, or finger movement is becoming more limited rather than gradually easier.

Seek urgent care if fingers become blue, pale, cold, or increasingly numb; you cannot move them; pain is severe and not settling with the advised plan; or the cast feels dangerously tight. New drainage, spreading redness, fever, or a sudden increase in swelling after another impact also needs prompt assessment.

Ask how often to elevate and move your fingers, how to check circulation around the cast, what pain changes are concerning, and whether the splint needs adjustment. Clarify when the next examination or imaging appointment is due and which activities remain restricted.
