When to worry about fever with measles

Worry about fever with measles when it is worsening, returns after improving, or comes with breathing trouble, confusion, severe dehydration, or a seizure. Fever is a very common presentation of Measles. It can occur with cough, runny nose, red eyes, feeling unwell and a spreading rash. A fever that settles and then returns, or a person who becomes more unwell after seeming to improve, can signal a complication such as an infection affecting the lungs or another part of the body. Fever by itself does not identify the cause, so symptoms and examination matter.

Rest, offer regular fluids, and use light clothing and a comfortably cool room. Watch for reduced drinking, dry mouth, very little urine, or unusual sleepiness, especially in a child. Contact a healthcare service before arriving if measles is suspected, so they can advise on safe arrangements and reduce contact with others. Use fever medicine only as directed on the label or by a clinician, taking account of age and other medicines.

Seek urgent medical care for difficulty breathing, blue or grey lips, a seizure, severe headache with a stiff neck, confusion, inability to stay awake, or signs of severe dehydration. Get same-day clinical advice when fever is getting worse, persists with marked weakness, returns after improvement, or is accompanied by chest pain, ear pain, or a worsening cough. Lower the threshold for medical advice for babies, pregnant people, and anyone with a weakened immune system.

Ask whether the pattern of fever suggests a complication, what changes should prompt urgent reassessment, and how to keep the person hydrated. It is also useful to ask how long to avoid close contact with others and whether household members need individual advice about measles exposure.
