When to worry about fever with croup

What fever can mean
Fever is a common presentation of Croup. Croup also causes swelling around the voice box and windpipe, which can produce a barking cough, hoarse voice, or a harsh sound when breathing in. The breathing pattern matters more than the fever reading alone.
A child may breathe noisily when upset and more quietly after settling. Noise that continues while resting, increasing effort, or a child who cannot speak, cry, or drink normally needs prompt assessment. Keep watching during the illness because symptoms can change, particularly overnight.
What to do now
Keep the child calm and upright on your lap or in a comfortable position. Offer frequent sips of fluid and check that they remain responsive. Use fever relief only as directed for the child's age and weight, and avoid steam or very hot air, which can cause burns.
Do not press a spoon into the mouth or examine the throat if the child is struggling to breathe or drooling. Note when the fever began, how it was measured, and whether the cough or breathing is changing.
When to seek care
Get urgent help if the child has noisy breathing while calm, ribs or neck pulling in, pauses in breathing, blue or pale skin, drooling with difficulty swallowing, unusual drowsiness, or cannot keep fluids down. Call emergency services if breathing is severely difficult, the child becomes limp, or you cannot wake them normally.
Contact a clinician the same day if fever is persistent, the cough is worsening, or the child is drinking much less or passing much less urine. Babies and children with chronic conditions may need earlier review.
