When to worry about fatigue with obstructive sleep apnoea

Short answer
Fatigue is a very common presentation of Obstructive Sleep Apnoea, but persistent daytime sleepiness needs assessment and treatment review.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance

- Repeated airway blockage fragments sleep.

- Daytime sleepiness can make driving unsafe.
Why sleep apnoea causes fatigue
In obstructive sleep apnoea, the upper airway narrows or closes repeatedly during sleep. The brain briefly responds to restore airflow, so sleep becomes fragmented even when you do not fully remember waking. This can cause morning headaches, dry mouth, poor concentration, irritability, and strong daytime sleepiness.
Fatigue may continue when treatment is not yet effective, equipment is uncomfortable, or another problem is affecting sleep. Snoring and witnessed pauses can be useful clues, but only a clinical assessment can establish the cause of daytime tiredness.
Practical next steps
Use prescribed sleep-apnoea treatment as directed and report mask leaks, dryness, pressure problems, or difficulty using the device. Keep regular sleep hours, avoid alcohol or sedating medicines unless a clinician has advised them, and do not drive when sleepy.
Arrange a review if fatigue remains after starting treatment, returns after improving, or interferes with work and daily tasks. The clinician may check treatment use, sleep quality, medicines, mood, and other causes of tiredness.
When to seek care
Seek urgent help for chest pain, severe breathing difficulty, fainting, new confusion, or sudden neurological symptoms. Stop driving and seek prompt advice if you struggle to stay awake at the wheel or have had a near-miss caused by sleepiness.
Contact your sleep team or clinician soon if daytime sleepiness is worsening, you wake choking repeatedly, or treatment is causing significant discomfort or is no longer controlling symptoms.
Questions for your doctor
Ask whether your treatment is controlling airway blockage, how to improve comfort and regular use, and whether another sleep or health problem could explain ongoing fatigue. Mention snoring, witnessed pauses, morning headaches, medication use, and any driving concern.
