When should you worry about fatigue if you have HIV infection?

Short answer
Fatigue is a common presentation of HIV Infection, but new, worsening, or disabling tiredness needs review, particularly with fever, breathlessness, confusion, or fainting.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common symptom
- Fatigue can accompany HIV Infection.
- Watch for
- Fever, breathlessness, confusion, or new weakness.
- Treatment
- Discuss medicine effects before making changes.
Why fatigue can happen
HIV infection can affect energy through immune activity, infection-related illness, sleep disruption, or reduced appetite. Anaemia, thyroid disease, low mood, and medicine effects may also cause tiredness. Fatigue by itself cannot show whether HIV is controlled or identify its cause; the pattern and accompanying symptoms matter.
Steps that may help
Note the timing of fatigue, sleep quality, appetite, fever, weight change, pain, and any recent medicine changes. Use gentle pacing, regular food and fluids, and short rest periods rather than pushing through severe exhaustion. Arrange a review when fatigue lasts, interferes with daily tasks, or follows a treatment change. Continue prescribed antiretroviral medicine unless your HIV care team tells you otherwise.
When care is urgent
Seek urgent help for trouble breathing, chest pain, fainting, new confusion, severe weakness, persistent vomiting, inability to drink, a severe headache with fever, or rapidly worsening illness. Contact your HIV clinician promptly for ongoing fever, night sweats, new diarrhoea, unexplained weight loss, or fatigue that is steadily increasing.
Questions to bring
Ask whether blood counts, thyroid testing, infection checks, sleep review, or a medicine review could clarify the problem. Ask how much activity is suitable, how to handle missed doses, and which symptoms should trigger same-day advice.
