When to worry about fatigue with heart failure

Heart failure can reduce the heart's ability to meet the body's demands. You may feel tired during activities that were previously manageable, need more rest, or notice reduced stamina. Breathlessness, ankle swelling, a cough, and rapid changes in weight can occur alongside worsening fluid congestion.

Fatigue can also result from anaemia, infection, poor sleep, low mood, or medicine effects. A change from your usual baseline matters, especially if it appears with breathing difficulty or swelling.

Follow your heart-failure plan, take medicines as prescribed, and record symptoms and any monitoring your care team has asked you to track. Balance activity with rest and avoid making major changes to fluids or medicines without advice.

Contact your heart-failure team or clinician if fatigue is increasing, daily tasks are harder, sleep is disturbed by breathing, or swelling is new. A review may identify a treatment adjustment or another cause.

Call emergency services for severe breathlessness at rest, chest pressure, fainting, blue or grey lips, confusion, or coughing up pink frothy fluid. Seek urgent clinical advice for a rapid change in breathlessness, swelling, or energy.
