When to worry about fatigue with coeliac disease

Short answer
Fatigue is a common presentation of Coeliac Disease, but arrange medical advice if it persists despite treatment or comes with fainting, breathlessness, bleeding, or severe weakness.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Active coeliac disease can be associated with fatigue.
- Watch for
- Persistent tiredness, fainting, breathlessness, or gastrointestinal bleeding.
What fatigue can mean
Coeliac disease can damage the small bowel lining, making it harder to absorb nutrients needed for energy. Iron, folate, or vitamin B12 deficiency may contribute, while ongoing gluten exposure, poor sleep, thyroid disease, infection, or another condition can also cause tiredness. Read more about Coeliac Disease and Fatigue.
Fatigue that gradually improves after an agreed gluten-free plan is different from exhaustion that remains unchanged, returns, or affects concentration and daily tasks. Weight loss, mouth ulcers, diarrhoea, abdominal symptoms, or pale skin are useful details to mention during a review.
What to do now
Contact your clinician or dietitian if fatigue continues, worsens, or makes ordinary activities difficult. Follow your prescribed dietary plan carefully and avoid self-diagnosing accidental exposure. Keep a note of meals, bowel symptoms, sleep, weight change, dizziness, and medicines. Do not begin iron or vitamin supplements before discussing testing, since the right treatment depends on the deficiency and its cause.
When to seek care urgently
Seek emergency help for fainting, severe breathing difficulty, chest pain, confusion, black or bloody stools, or weakness that prevents safe movement. Request prompt medical advice for repeated vomiting, inability to drink, marked dizziness, rapid weight loss, or fatigue that becomes suddenly intense. Same-day assessment is sensible if dehydration symptoms accompany ongoing diarrhoea.
Questions for your doctor
Ask whether tests should assess iron stores, blood counts, folate, vitamin B12, thyroid function, and ongoing coeliac activity; whether your diet needs specialist review; and what symptoms should trigger urgent help.

Why can coeliac disease cause tiredness?
Reduced absorption of nutrients, particularly those involved in making healthy blood cells, can reduce energy.
Should I take iron for fatigue?
Ask a clinician to check the cause first; supplements are not suitable for every person or situation.
