When to worry about earache with earwax buildup

A plug of wax can press against the sensitive ear canal or eardrum and create fullness, reduced hearing, ringing, or an ache. Similar symptoms can come from infection, a foreign object, or eardrum irritation, so pain alone cannot confirm that wax is the cause. Looking into the ear is often needed.

Do not insert cotton buds, hairpins, or other objects into the ear. They can push wax deeper or injure the canal. Ask a pharmacist or clinician whether a softening product is suitable; avoid drops if you may have a perforated eardrum, ear tubes, or previous ear surgery unless advised. Professional removal may be safer.

Seek prompt assessment for discharge, blood, fever, severe tenderness, sudden hearing loss, marked dizziness, or pain that persists after wax is removed. Get urgent help for facial weakness, swelling behind the ear, severe headache, confusion, or rapidly worsening symptoms. People with diabetes or reduced immunity should seek advice sooner for painful ear symptoms.

Ask whether wax is blocking the view of the eardrum, which removal method is appropriate, and what symptoms would suggest infection. Mention hearing changes, previous ear procedures, skin conditions, attempts at cleaning, and all drops used.
