When to worry about dry skin with asteatotic eczema

Short answer
Dry Skin is a very common presentation of Asteatotic Eczema, but deep cracks, spreading redness, discharge, or severe discomfort needs assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common area
- Lower legs, particularly when the skin barrier is very dry
- Barrier care
- Short lukewarm washing followed by thick fragrance-free moisturiser
- Warning signs
- Heat, spreading redness, pus, deep cracks, fever, or worsening pain
What the dryness can mean
Asteatotic eczema happens when the skin barrier becomes very dry and irritated. The skin may feel tight, rough, flaky, or itchy, with fine cracks that can form a network-like pattern, especially on the lower legs. Cold, dry air, hot bathing, strong cleansers, and frequent washing can worsen the barrier.
Mild flaking is different from skin that is weeping, crusted, hot, or increasingly painful. Those changes may indicate infection or a stronger inflammatory flare and should be assessed rather than managed only with moisturiser.
What you can do now
Use a fragrance-free, thick moisturiser soon after a short lukewarm wash and reapply when the skin feels dry. Choose gentle, non-foaming cleansing and pat the skin dry. Avoid scratching, rough exfoliation, fragranced products, and very hot water.
If cracks persist, itch disrupts sleep, or emollient care is not enough, arrange a clinician review. A clinician may recommend an anti-inflammatory treatment and check for triggers, medicines, or another cause of persistent scaling.
When to seek care
Book an appointment for worsening redness, warmth, swelling, pain, deep fissures, bleeding, yellow crust, or fluid from the skin. Seek prompt care for rapidly spreading redness, fever, or feeling unwell. People with diabetes, poor circulation, or reduced immunity should seek advice early when cracks or wounds develop.
Questions for your doctor
Ask whether the pattern fits asteatotic eczema, which moisturiser texture and washing routine suit you, and whether a prescription cream is needed. Ask how to protect cracks, how to recognise infection, and when follow-up should occur if the skin does not settle.

Questions people ask
Why does asteatotic eczema crack?
Very dry skin loses flexibility and becomes easier to fissure, especially when washing, cold air, or friction further weakens the barrier.
Can moisturiser treat an infected crack?
Moisturiser supports the barrier but cannot replace assessment and treatment for infection signs such as spreading redness, warmth, pus, or fever.
