When to worry about difficulty swallowing with oesophageal cancer

Short answer
Difficulty Swallowing is a very common presentation of Oesophageal Cancer. Tell your cancer team about any new or worsening change, and seek urgent help if you cannot swallow saliva, choke, or develop breathing difficulty.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Emergency sign
- Cannot swallow saliva or breathe normally
What the change may mean
A tumour or treatment-related inflammation can narrow the passage through the oesophagus, so food may feel slow, lodged, or painful. Some people notice a change first with solid foods and later with softer foods or drinks. Reflux and other conditions can also affect swallowing, so the symptom needs evaluation rather than self-diagnosis.
What to do
Contact your oncology or specialist team as soon as you notice a new pattern, reduced intake, coughing during meals, or difficulty taking medicines. Follow the texture and nutrition advice in your care plan. Take small mouthfuls, eat slowly, and sit upright. Do not stop cancer treatment or prescribed medicines without speaking with the team.
When to seek care
Request prompt advice for worsening swallowing, painful swallowing, repeated vomiting, dehydration symptoms, marked weakness, bleeding, or unintended weight loss. Use emergency services for choking, severe breathing trouble, a complete blockage, or inability to swallow saliva.
Questions for your doctor
Ask what may be causing the change, whether an endoscopic or imaging assessment is needed, and how to maintain fluids and nutrition safely. Ask which food textures are suitable, how to manage tablets, and whom to contact outside clinic hours.
