When to worry about difficulty swallowing with hiatus hernia

A hiatus hernia can worsen reflux and irritation in the lower oesophagus. Swelling, spasm, or narrowing from long-standing irritation may make food feel slow to pass. The sensation may come and go, but a change from your usual pattern deserves assessment. Difficulty swallowing does not by itself establish the cause.

Arrange a clinical review when swallowing becomes difficult or starts happening more often. Until then, take small bites, chew thoroughly, choose softer foods if needed, and remain upright after meals. Do not force food past a sticking sensation. Record whether solids, liquids, or both cause trouble and mention reflux, chest discomfort, vomiting, or weight change.

Contact a clinician promptly for progressive difficulty, food sticking, painful swallowing, repeated vomiting, black stools, vomiting blood, or unplanned weight loss. Seek emergency help if food is completely stuck, you cannot swallow saliva, you are choking, or breathing is difficult.

Ask whether reflux or the hernia could explain the symptom, whether you need an examination or swallowing test, and which changes should trigger urgent review. Ask how to take any prescribed reflux treatment and whether your current medicines could irritate the oesophagus.
