When to worry about difficulty swallowing with head and neck cancer

Short answer
Difficulty Swallowing is a common presentation of Head and Neck Cancer. Report new, worsening or painful swallowing promptly, especially when coughing, weight change or treatment-related symptoms are present.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Report promptly
- New, worsening or painful swallowing
- Airway clue
- Coughing or a wet voice during meals
What swallowing difficulty may reflect
Head and Neck Cancer can affect structures used to move food and liquid from the mouth to the throat and oesophagus. Swallowing may become painful, slow or less coordinated, and treatment can also cause dryness, swelling, soreness or changes in muscle function. Coughing during meals, a wet or gurgly voice, food remaining in the mouth and repeated chest infections can signal that swallowing is not protecting the airway well. The exact cause needs review by the treating team.
What to do
Tell your oncology or ear, nose and throat team about any change rather than waiting for the next routine visit. Keep a record of foods that cause trouble, coughing during meals, pain, voice changes and how much you can drink. Follow any texture, posture or swallowing exercises provided by your speech and language therapist. Take small bites, stay upright and pause if coughing starts; do not force food or drink through a blockage.
When to seek care
Seek urgent help if you cannot swallow saliva, have sudden breathing difficulty, choking that does not settle, blue lips, severe weakness or confusion. Contact the cancer care team promptly for progressive difficulty, dehydration, repeated coughing with drinks, bleeding, persistent pain, or unplanned weight change. Early review can help protect nutrition, hydration and the airway.
Questions for your doctor
Ask whether a swallowing assessment is needed, whether treatment effects could explain the change, and which food textures and exercises are safest. Discuss pain relief, mouth dryness, nutrition support and signs that should trigger an emergency call.
