When to worry about difficulty passing urine with benign prostatic hyperplasia

As the prostate enlarges, it can narrow the outlet from the bladder. You may need to wait before the stream starts, pass a weaker stream, stop and start, strain, dribble, or feel that the bladder has not emptied. Night-time urination and urgency can occur as well.

These symptoms can also occur with infection, medicines, narrowing of the urethra, or other bladder problems. A clinician may examine you, test a urine sample, review medicines, and assess how well the bladder empties.

Arrange a routine review when flow changes persist, disturb sleep, or limit activities. Note the stream, urgency, leakage, night-time trips, pain, and any visible blood. Do not stop prescribed medicines or sharply restrict fluids without medical advice; some medicines for colds, allergies, or mood can worsen urinary difficulty.

Take your medication list to the appointment. Treatment may involve monitoring, medicine, or a procedure depending on symptoms, examination findings, and bladder function.

Seek urgent care if you cannot pass urine, develop increasing lower abdominal pain or swelling, have fever or chills with urinary symptoms, or see blood in the urine. Vomiting, confusion, severe weakness, or pain in the side or back alongside urinary difficulty also warrants prompt assessment.

Ask whether your symptoms suggest incomplete emptying, which medicines may be contributing, and whether urine testing or a bladder scan is needed. Ask how to recognise retention and when a treatment review should happen.
