When to Worry About Daytime Sleepiness With Obstructive Sleep Apnoea

Short answer
Daytime Sleepiness is a very common presentation of Obstructive Sleep Apnoea, but persistent or dangerous drowsiness needs prompt medical review.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Obstructive Sleep Apnoea commonly causes daytime sleepiness through fragmented sleep.

- Ongoing drowsiness despite treatment should be discussed with a sleep clinician or doctor.
Why sleep apnoea can cause sleepiness
In obstructive sleep apnoea, the upper airway narrows or closes repeatedly during sleep. Breathing interruptions and brief arousals can fragment sleep, even when you do not remember waking. The result may be poor alertness, slower reactions, difficulty concentrating, irritability and unplanned dozing during the day.
Sleepiness can remain when treatment is not being used consistently, the device or mask is uncomfortable, the settings need review, or another problem is also affecting sleep. Snoring, gasping, witnessed breathing pauses, dry mouth or morning headaches are useful details to report.
Steps to take
Use prescribed sleep-apnoea treatment as directed and discuss mask leaks, dryness, discomfort or difficulty using it rather than abandoning treatment. Avoid alcohol or sedating medicines unless a clinician has advised that they are suitable, because they may worsen breathing or alertness. Keep a consistent sleep routine and allow enough time for sleep.
Do not drive, use machinery or perform safety-critical work when drowsy. If sleepiness continues despite treatment, contact your sleep clinician or doctor; they may review treatment data, medicines, sleep duration and other causes of fatigue.
When to seek care
Seek prompt review if you fall asleep unintentionally, have a near-miss while driving, or remain sleepy despite using treatment. Arrange medical advice soon for worsening snoring or witnessed breathing pauses, morning headaches, poor concentration, mood change or difficulty keeping up with ordinary activities.
Get urgent help for severe breathing difficulty while awake, chest pain, fainting, sudden severe confusion or new one-sided weakness. Do not assume these symptoms are simply sleep apnoea or tiredness.
Questions for your doctor
Ask: Is my treatment controlling the breathing interruptions? Could mask fit, device use, medicines, alcohol or another sleep disorder explain the remaining sleepiness? Do I need a sleep study or treatment adjustment? Also ask when it is safe for you to drive and what warning signs should trigger urgent help.
