When to worry about coughing up phlegm with COPD

COPD can produce mucus because irritated airways make and retain more secretions. Phlegm that becomes thicker, darker, or more plentiful may occur during a flare, especially when wheezing, chest tightness, or breathlessness also increases. Blood-streaked mucus can have several causes and should be assessed rather than dismissed.

Notice the colour and amount of sputum, your breathing compared with usual, and whether fever or tiredness has appeared. Follow your personal COPD action plan if you have one, use prescribed medicines exactly as directed, and drink fluids unless a clinician has restricted them. Do not start leftover antibiotics or steroids without advice.

Contact a clinician promptly for a clear change in phlegm with worsening breathlessness, fever, chest discomfort, or reduced ability to do usual activities. Seek emergency help for severe difficulty breathing, blue or grey lips, confusion, fainting, or more than a small amount of blood.
