When to worry about coughing up phlegm with bronchiectasis

Bronchiectasis widens and damages parts of the airways, making mucus harder to clear. People may notice daily phlegm, but an increase in volume, thickness, colour, smell, cough, fatigue, wheeze, or breathlessness can point to infection or worsening airway inflammation. Blood in sputum should also be reported.

Follow the airway-clearance routine taught by your respiratory team, such as breathing exercises or prescribed devices. Drink enough fluid for your needs and record changes in sputum and breathing. Use rescue medicines or antibiotics only according to your personal action plan; do not borrow treatment from someone else.

Contact your respiratory clinician when phlegm suddenly increases or changes, fever develops, energy falls, or breathlessness is worse than usual. Seek emergency care for heavy or ongoing bleeding, severe breathlessness, chest pain, fainting, blue lips, or confusion. Bring your medication list and action plan.

Ask what sputum changes define your flare plan, how to collect a sample, and when antibiotics or inhalers are appropriate. Clarify your usual breathing baseline, the emergency threshold, and whether your airway-clearance technique needs checking.
