When to worry about coughing up blood with tuberculosis

Tuberculosis can inflame and injure airways or lung tissue, allowing blood to mix with mucus. Blood-streaked sputum may occur alongside a cough, fever, night sweats, tiredness, chest discomfort, or weight loss. The symptom does not show how extensive the infection is, so assessment remains necessary even when the amount seems small.

Contact the tuberculosis team or a clinician promptly and describe the colour, amount, and frequency of blood. Continue tuberculosis medicines exactly as prescribed unless your care team instructs otherwise. Cover your mouth when coughing, dispose of tissues safely, and follow any infection-control advice you have been given.

Seek emergency help for more than a trace of blood, bleeding that will not settle, severe breathing difficulty, chest pain, fainting, confusion, or blue lips. Sit upright while waiting for help and avoid driving if weak or breathless. Tell responders that you have tuberculosis and list your medicines.

Ask whether the bleeding suggests airway irritation, treatment difficulty, or another lung problem; whether your medicines need review; and how to protect household contacts. Confirm which symptoms require emergency care and when your next sputum or lung assessment is due.
