When to worry about chest pain if you have peptic ulcer

A peptic ulcer is an open sore in the lining of the stomach or upper small intestine. It commonly causes burning or aching in the upper abdomen and may be felt near the lower chest. Symptoms can relate to meals, hunger, or indigestion, but the location and timing cannot confirm the cause.

Chest pain may also arise from the heart, lungs, muscles, or oesophagus. Ulcers can bleed or perforate, causing a sudden change in symptoms and requiring urgent care.

Rest and avoid strenuous activity. Take ulcer treatment only as prescribed, and do not add anti-inflammatory painkillers without medical advice because they can irritate the stomach and contribute to bleeding. Record when pain occurs, its relation to food, and any vomiting or stool changes.

Contact a clinician for recurring discomfort, symptoms that are worsening, or pain that continues despite treatment. Bring a list of medicines, including non-prescription products.

Call emergency services for severe or persistent chest pain, pressure with breathlessness, fainting, sweating, nausea, or pain spreading to the arm, back, neck, or jaw. Seek urgent help for vomiting blood, black or tar-like stools, sudden severe abdominal pain, a hard or swollen abdomen, confusion, or collapse. Do not drive yourself if seriously unwell.

Ask whether your symptoms could be ulcer-related, whether your medicines increase bleeding risk, and how to recognise a complication. Ask what tests or follow-up are appropriate if pain returns or treatment does not help.
