When to worry about buttock pain with piriformis syndrome

Buttock Pain is a very common presentation of Piriformis Syndrome. The piriformis is a deep muscle near the hip; irritation in this area can affect the nearby sciatic nerve. Pain may feel deep in one buttock and become sharper with prolonged sitting, climbing, running, or rotating the hip. Tingling or aching down the back of the thigh can occur. Constant numbness, clear loss of strength, or symptoms in both legs need broader evaluation.

Temporarily reduce activities that reproduce the pain, take regular standing breaks during sitting, and avoid aggressive stretches that send pain down the leg. A physiotherapist can assess the hip, lower back, gait, and nerve function before selecting exercises. Record what triggers the pain and whether sensation or strength changes.

Go to urgent care after a substantial injury if you cannot walk or bear weight. Seek prompt assessment for rapidly worsening pain, new foot or leg weakness, spreading numbness, fever, a hot swollen area, or bladder and bowel changes. Emergency symptoms should not be attributed to piriformis syndrome without examination.

Ask whether the symptoms fit piriformis syndrome or could arise from the spine or hip. Ask how strength and sensation will be checked, which activities to modify, and what changes require emergency review.
