When to Worry About Blood in Urine With Prostate Cancer

Blood may come from the prostate, bladder, kidneys, or urethra. Prostate cancer itself can cause urinary bleeding, while infection, a stone, enlarged prostate tissue, recent instrumentation, or cancer treatment can also contribute. The colour and amount do not show the cause reliably. Clots can interfere with urine flow, creating pressure, pain, or retention.

Contact your oncology or urology team promptly about any visible blood, even if it stops. Record when it began, its colour, clots, pain, urinary stream, fever, and recent procedures. Take medicines as prescribed and ask before changing anticoagulants or other treatment. Do not wait for a routine cancer appointment if bleeding is increasing or urination is becoming difficult.

Attend emergency care if you cannot pass urine, pass large or repeated clots, have heavy or persistent bleeding, severe lower abdominal pain, faintness, confusion, fever, or shaking chills. These features can signal blockage, infection, or significant blood loss and need immediate assessment.

Ask whether urine testing, blood tests, imaging, or a bladder examination is appropriate. Clarify how your cancer treatment might affect bleeding, which medicines raise concern, and whom to contact outside clinic hours.
