When to worry about black stools with a peptic ulcer

Black Stools is a very common presentation of Peptic Ulcer. An ulcer can open a blood vessel in the stomach or upper intestine. As blood travels through digestion, it may make stool black and tar-like. The colour may be less obvious when bleeding is small, so a change from your usual bowel movement still matters.

Iron, bismuth, and certain foods can also darken stool. However, a peptic ulcer history raises the need for prompt clinical advice rather than self-diagnosis. Weakness, breathlessness, racing heartbeat, or increasing tiredness may suggest blood loss.

Contact urgent medical care and say that you have a peptic ulcer and black stool. Bring or photograph a list of medicines, including aspirin, blood thinners, steroids, and anti-inflammatory painkillers. Do not take an additional anti-inflammatory painkiller unless a clinician specifically tells you to.

Rest while arranging help, especially if you feel unsteady. Do not drive when dizzy. Keep track of further stool changes, vomiting, pain, and the time symptoms began.

Call emergency services for collapse, fainting, confusion, severe weakness, vomiting fresh blood, coffee-ground vomit, severe abdominal pain, or cold clammy skin. Seek same-day assessment for more than one black stool, persistent pain, or symptoms that are worsening. An examination can determine whether bleeding is active and what monitoring or treatment is needed.

Ask how your ulcer should be treated, whether your medicines increase bleeding risk, and whether you need blood tests or specialist assessment. Tell the clinician about prior bleeding and any change in your usual ulcer symptoms.
