What the swelling can mean
Gout happens when urate crystals irritate a joint, causing an inflammatory flare. The ankle may become swollen, warm, red, and very tender, sometimes with pain that makes even light contact uncomfortable. Other causes of a hot swollen joint include infection, injury, or another inflammatory condition. Because joint infection can damage the joint quickly, appearance alone is not enough to identify the cause.
What to do during a flare
Rest the ankle in a comfortable position and avoid pressure from tight footwear. Elevate it if that reduces throbbing. Drink normally unless a clinician has told you to restrict fluids, and take gout medicine only according to your prescribed plan. Do not start, stop, or change long-term urate treatment without advice. Keep track of when symptoms began, which joints are involved, and any fever.
When to seek care
Contact a clinician promptly for a first suspected gout attack, a flare that is unusually severe, or swelling that is not settling with your usual plan. Seek urgent care for fever, chills, rapidly spreading redness, pus, confusion, faintness, or feeling very unwell. Urgent review is also sensible when you cannot move the ankle or when symptoms began after an injury. Testing may be needed to distinguish crystals from infection.
