When to worry about ankle swelling with an ankle sprain

Short answer
Ankle swelling is a very common presentation of ankle sprain, but seek prompt care if it is worsening or paired with severe pain, numbness, colour change, deformity, or inability to bear weight.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Expected symptom
- Swelling and bruising can occur after ligament injury.
- Prompt review
- Worsening symptoms, bone tenderness, or difficult walking.
- Urgent warning
- Numbness, colour change, deformity, or breathlessness.
What the swelling can mean
A sprain stretches or tears ligaments around the ankle. The injured tissues can trigger inflammation, fluid movement, bruising, warmth, and stiffness. Swelling may be more noticeable after standing and may settle with rest and elevation. Swelling alone cannot show how serious the ligament injury is, and an ankle fracture or another injury can sometimes look similar.
What you can do now
Protect the ankle from movements that increase pain. Rest from running and jumping, elevate the foot when sitting, and use a cold pack wrapped in cloth for short periods if it feels helpful. A comfortable supportive shoe or clinician-advised brace may reduce strain. Gentle movement within a tolerable range can help prevent stiffness; do not force the joint or massage a newly bruised area.
When to seek care
Arrange medical assessment if pain or swelling keeps increasing, walking remains very difficult, the ankle looks deformed, or tenderness is directly over a bone. Seek urgent help for a cold or blue foot, new numbness, severe pressure, an open wound, or sudden calf swelling with breathlessness or chest pain. A clinician may examine stability and decide whether imaging or protected weight-bearing is needed.
Questions for your doctor
Ask: Could this be a fracture as well as a sprain? Should I use a brace or crutches? Which movements are safe today? When should I begin strengthening and balance exercises? What change in swelling should trigger a review?

Questions people ask
Is swelling after an ankle sprain normal?
Yes. Swelling is a very common presentation of ankle sprain, although its pattern and severity should gradually improve rather than steadily worsen.
Should I walk on a swollen ankle?
Only as much as feels tolerable and safe. Use support if advised, and obtain assessment when weight-bearing causes marked pain or remains difficult.
