When to worry about ankle swelling with an ankle fracture

Why swelling happens
A fracture can irritate nearby tissues and cause swelling and bruising around the ankle and foot. Swelling may increase when the leg hangs down. It should not be treated as harmless when it becomes tight, rapidly worsens, or comes with new pain or altered sensation. A cast or splint that feels increasingly tight can press on skin, nerves, or blood vessels. Ankle swelling alone cannot show how the bone is aligned or how tissues are healing.
Protect the ankle
Keep the ankle supported as instructed and do not put weight through it unless your clinician has allowed that. Elevate the leg when resting, keep the support positioned as advised, and keep a cast or dressing dry. Check the toes for movement, sensation, warmth, and colour without disturbing the injury. Follow the fracture team’s review plan even if swelling begins to settle.
When to seek urgent care
Contact urgent services for severe or escalating pain, a dangerously tight cast or splint, new numbness or weakness, toes that become cold, blue, or pale, or swelling that increases quickly. Seek immediate assessment for an open fracture, obvious deformity, or uncontrolled bleeding. Sudden calf pain or breathlessness after an injury also requires emergency attention.
