When to worry about abdominal pain with a urinary tract infection

A bladder infection can cause pressure or aching low in the abdomen, often with burning urine, frequent urges, or cloudy-smelling urine. Pain higher in the side or back can signal that infection has reached the kidneys and deserves quicker assessment. Abdominal pain can also have another cause, so its location, timing, and associated symptoms matter.

Arrange clinical advice if urinary symptoms and abdominal pain occur together, especially in a child, during pregnancy, or in someone with a weakened immune system. Drink normally unless a clinician has advised fluid restriction, rest, and take only suitable pain relief according to the label or a pharmacist’s advice. Do not use leftover antibiotics or stop a prescribed course without guidance.

Seek urgent care for intense or rapidly increasing pain, pain beneath the ribs in the back or side, fever with shaking chills, repeated vomiting, faintness, new confusion, very little urine, or inability to keep fluids down. If symptoms are mild but do not start improving after treatment, contact the prescriber for review.

Ask whether the symptoms fit a bladder or kidney infection, whether a urine test is needed, which pain medicines are appropriate, and what improvement should look like. Mention pregnancy, allergies, kidney problems, previous infections, and every medicine being taken.
