Short answer

Abdominal Pain is a common presentation of Lactose Intolerance, but severe, persistent, or unusual pain needs medical assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common pattern
Cramping and bloating after lactose-containing food
Needs review
Pain that is persistent, worsening, or unrelated to meals

What the pain may mean

Lactose intolerance can cause cramping, bloating, rumbling, wind, and loose stools after milk or other lactose-containing foods. Symptoms often settle when the trigger has passed. Pain that occurs without a food pattern, keeps returning despite avoiding lactose, or steadily worsens may have another explanation. A new diagnosis should not be assumed from pain alone.

What to do now

Record when the pain starts, what you ate, bowel changes, and any vomiting or fever. Follow an agreed lactose-reduction plan without removing nutritious foods unnecessarily. Do not repeatedly test yourself with large amounts of milk. Arrange a clinician review if symptoms continue, interfere with eating or sleep, or you are losing weight.

When to seek care

Seek urgent care for sudden severe pain, a hard or swollen abdomen, repeated vomiting, fainting, confusion, blood in the stool, black stools, or fever with worsening pain. Get prompt advice if you cannot keep fluids down or pass stool and wind. These features are not explained safely by lactose intolerance without an examination.

Questions for your doctor

Ask whether your pattern fits lactose intolerance, whether another condition should be checked, and how to maintain calcium and vitamin intake while changing your diet.
