When to worry about abdominal pain with gallstones

A gallstone can obstruct the flow of bile and cause pain in the upper right or upper middle abdomen. Discomfort may follow a meal and can spread toward the back or right shoulder. Pain that lasts, returns frequently, or is joined by fever or yellowing of the eyes may reflect inflammation or blockage rather than a brief episode.

Stop activity, rest, and take small sips of fluid if you are not vomiting. Avoid foods that clearly trigger symptoms until you have medical advice. Record the start time, location, severity, meals, vomiting, urine colour, and stool colour. Do not take another person’s prescription medicine.

Go to urgent care for severe or unrelenting pain, fever or chills, yellow skin or eyes, dark urine, pale stools, repeated vomiting, faintness, confusion, or a swollen abdomen. Call emergency services if you collapse, have trouble breathing, or cannot stay awake. New pain should be assessed even when you already know you have gallstones.

Ask whether your symptoms suggest a blocked bile duct or inflamed gallbladder, whether ultrasound or blood tests are appropriate, and how to reduce further attacks. Share your meal triggers, previous scans, medicines, and any change in skin or urine colour.
