When to Worry About Abdominal Pain With Crohn's Disease

Short answer
Seek urgent medical care for severe or worsening abdominal pain, a swollen or rigid abdomen, repeated vomiting, inability to pass stool or gas, fever with marked illness, fainting, or blood in the stool.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Cramping may occur with active Crohn's inflammation and bowel changes.
- Urgent warning
- Severe pain with vomiting, swelling, or inability to pass stool or gas needs urgent care.
- Useful record
- Track location, timing, bowel movements, vomiting, fever, bleeding, and fluid intake.
What the pain may mean
Most people with Crohn's Disease notice abdominal pain, but its pattern matters. Cramping with diarrhoea, urgency, tiredness, or reduced appetite can occur during active bowel inflammation. Pain that steadily intensifies, becomes focused in one area, or comes with vomiting can point to a complication such as narrowing of the bowel, an abscess, or another abdominal illness. Pain can also come from trapped gas, constipation, infection, or a medicine-related problem, so a new pattern should not be self-diagnosed as a flare.
What to do now
Record when the pain began, where it is, whether it comes in waves, and how it relates to meals or bowel movements. Note diarrhoea, constipation, vomiting, fever, bleeding, and whether you can drink and pass urine. Follow your Crohn's treatment plan, drink small amounts if you can keep fluids down, and contact your gastroenterology team promptly if the pain is new, persistent, or different from your usual symptoms. Do not start anti-inflammatory pain medicines or change steroid treatment without advice.
When to seek care
Go to an emergency department for sudden severe pain, a hard or visibly swollen abdomen, repeated vomiting, inability to pass stool or gas, heavy rectal bleeding, black stools, fainting, confusion, or signs of dehydration such as very little urine and severe dizziness. Seek same-day medical advice for persistent pain with fever, worsening diarrhoea, reduced fluid intake, or pain that repeatedly wakes you. If symptoms are mild but continue beyond your usual pattern, arrange a review rather than waiting for the next routine appointment.
Questions for your doctor
Ask whether your symptoms fit active Crohn's inflammation or need assessment for narrowing, infection, or another cause. You can also ask which pain relief is suitable for you, when to contact the specialist team, whether your medicines need checking, and which tests might clarify the cause. Bring a list of medicines, recent bowel changes, and any home temperature readings.
