When should you see a doctor about ankle swelling?

Short answer
See a doctor about ankle swelling when it follows an injury, is sudden or severe, affects walking, keeps returning, or comes with redness or fever.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Arrange review when
- Swelling persists, recurs, or limits walking
- Urgent symptoms
- Deformity, fever, severe pain, chest symptoms, or a cold numb foot
What swelling can mean
Swelling is a response to irritation, injury, or fluid collecting around the ankle. An ankle sprain can swell after a twist, while an ankle fracture may cause marked swelling, bruising, and difficulty bearing weight. Ankle osteoarthritis may produce swelling with stiffness after activity. Gout often presents with a rapidly painful, hot, and red joint. The appearance alone cannot confirm the cause, particularly when there was no obvious injury.
Steps while arranging care
After a minor injury, reduce weight-bearing, elevate the leg, and use a wrapped cold pack for short periods if it is comfortable. Avoid tight footwear and do not massage a hot, very tender, or newly injured area. Note when the swelling began, whether one or both ankles are affected, and any new medicines, bites, travel, or injury. A clinician can use this history and an examination to decide whether tests are needed.
Urgent warning signs
Seek urgent care if swelling is accompanied by severe pain, a visible deformity, inability to walk, spreading redness, fever, chest pain, or shortness of breath. Prompt assessment is also sensible when the ankle is hot and intensely tender without an injury, or when swelling is rapidly increasing. Do not wait for home care to work when the foot becomes pale, blue, cold, or numb.
Questions for your doctor
Ask what may be causing the swelling, whether you need an X-ray or other test, how much activity is safe, and which symptoms should bring you back urgently. Tell the clinician about previous sprains and any history of gout.

Questions people ask
Is swelling after a twist always a fracture?
No. A sprain can also swell, but inability to bear weight, deformity, or severe pain needs prompt assessment.
Can I exercise with a swollen ankle?
Avoid impact activity while swelling and pain are increasing; ask a clinician when graded movement is appropriate.
