When is hydroquinone treatment recommended for perioral hyperpigmentation?

Darkening around the mouth may follow irritation, dryness, licking, hair removal, acne, or a rash. A clinician may first check for active inflammation because treating pigment while the skin is irritated can worsen the colour. The location also makes accidental contact with the lips and corners of the mouth more likely.

Hydroquinone reduces pigment production in the skin where it is applied. It does not treat every cause of perioral darkening, and it should not be used on cracked, peeling, infected, or actively inflamed skin unless a clinician gives clear instructions.

Use a small amount only on the prescribed darker skin, keeping it away from the lips, nostrils, and mouth corners. Wash hands after application. Use a bland moisturiser and avoid licking, rubbing, fragranced products, harsh scrubs, and unplanned acids or retinoids while the area settles.

Protect the face from sunlight with shade, clothing, and sunscreen that does not sting. Stop and contact the prescriber for burning, swelling, blistering, marked redness, peeling, or new unusual darkening. Do not apply a stronger product or increase frequency because the area looks slow to change.

Seek care for a spreading rash, persistent itch, pain, cracking, sores, crusting, or swelling around the mouth. Have a clinician examine darkening that appeared suddenly, changes despite gentle care, or comes with thickened skin or symptoms elsewhere. Prompt advice is needed if treatment reaches the lips or causes significant irritation.

Ask what triggered the perioral darkening, whether an active rash needs treatment first, and exactly how close to the lips the medicine may go. Confirm compatible moisturisers and sunscreen, the review time, and alternatives if irritation develops.
