When are topical corticosteroids recommended for guttate psoriasis?

Guttate psoriasis causes many small, raindrop-like patches, often across the trunk or limbs. The spots can be itchy and covered with scale. Because there may be many areas to treat, a clinician considers the extent of the eruption, the thickness of the plaques, and whether topical application is practical.

A corticosteroid reduces inflammation and can make individual spots less raised and irritated. It does not address every factor that may be involved in a flare. The plan may therefore include moisturising, another skin treatment, or a review if the eruption is extensive or persistent.

Follow the prescribed schedule and apply a thin layer to the psoriasis spots rather than unaffected skin. Wash hands after use unless the hands are the treatment area. Use moisturiser as advised, and avoid picking scale, which can further irritate the skin.

Do not use a stronger product, extend treatment, or cover the skin with a dressing without medical advice. Tell the clinician if the rash began after a sore throat or other infection, and mention any previous psoriasis treatment so the plan can be tailored.

Arrange medical review if the spots rapidly multiply, cover much of the body, become painful, or fail to settle with the agreed plan. Seek urgent care if skin becomes widespread and intensely red, if there are blisters or peeling, or if you feel severely unwell. New joint swelling also deserves prompt assessment.

Ask whether the pattern is definitely guttate psoriasis, which areas should receive the steroid, how long to use it, and what alternative is available if applying cream to many spots is difficult.
