When are topical calcineurin inhibitors recommended for vitiligo?

Vitiligo causes areas of skin to lose pigment, often creating lighter patches with clear borders. It is linked to immune activity affecting pigment-producing cells. Calcineurin inhibitors reduce local immune signalling and may support repigmentation in selected patches; response is gradual and varies by site and disease activity. Facial skin can respond differently from hands, feet, or areas with repeated friction, so the treatment plan is individual rather than based on the patch alone.

Apply the prescribed product to the affected skin exactly as directed, avoiding the eyes and broken skin. Mild warmth or irritation can happen at first; tell your clinician if it is persistent or intense. Protect depigmented areas from sun exposure with clothing and a suitable sunscreen, because they can burn more easily and the contrast may become more noticeable after tanning. Keep follow-up appointments so colour change and new patches can be assessed.

Seek medical advice if light patches spread quickly, involve the eyelids or lips, become painful, or develop scaling, crusting, or sores. A changing rash is not automatically vitiligo, and infection or another skin condition may need different treatment. Report eye symptoms or significant treatment irritation promptly.

Ask whether the patches are consistent with vitiligo, which sites are suitable, how long to trial treatment, how progress will be recorded, and whether light-based treatment should be considered.
