When Is Topical Antifungal Treatment Recommended for Cradle Cap?

Cradle cap, also called infantile seborrheic dermatitis, often causes greasy or dry scales on the scalp. Mild cases may improve with gentle washing and softening of scales. A clinician may consider an antifungal when the scale is widespread, keeps returning, or affects nearby skin such as the eyebrows or skin folds. Not every flaky scalp is cradle cap, and inflamed or oozing skin can point to another problem.

Use only a product and schedule recommended for your baby’s age. Apply it carefully as directed and keep it away from the eyes and mouth. Wash with a mild baby cleanser, soften adherent scale before gentle brushing, and never pick or scrape the scalp. Stop and seek advice if the skin becomes more inflamed after application. Avoid adult scalp products unless a paediatric clinician specifically says they are suitable.

Contact a clinician if the scalp is very red, swollen, painful, cracked, bleeding, or leaking fluid, or if the rash spreads beyond the scalp. Seek urgent help if your baby has fever, seems unusually sleepy, feeds poorly, or appears seriously unwell. A persistent rash that does not respond to the advised routine also warrants review.

Ask whether the appearance is consistent with cradle cap, why an antifungal is being considered, and where it should be applied. Confirm the suitable amount and duration, what to do after a missed application, and which moisturising or washing products should be avoided.
