When is thyroid hormone replacement recommended for hypothyroidism?

Short answer

Thyroid hormone replacement is recommended when tests and clinical assessment show that the body needs additional thyroid hormone, with the dose reviewed over time.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How replacement works

Hypothyroidism means the thyroid does not provide enough hormone for the body's needs. Replacement medicine supplies a form of thyroid hormone so the clinician can restore an appropriate level. Treatment decisions depend on test results, symptoms, age, other health conditions and pregnancy or pregnancy plans. A mildly abnormal result may need repeat testing or observation rather than immediate medicine.

Using treatment safely

Take the medicine exactly as prescribed and keep the dose consistent from day to day. Ask how to separate it from food, iron, calcium or other medicines if relevant, because absorption can be affected. Do not change the dose or stop treatment because symptoms improve. Attend planned blood-test reviews; the clinician may adjust treatment after the body has had time to respond or when health circumstances change.

When to get medical help

Seek urgent care for chest pain, severe shortness of breath, fainting or a very fast or irregular heartbeat. Arrange prompt advice for worsening swelling, profound sleepiness, confusion or a noticeable change after a dose adjustment. Tell your clinician early about pregnancy, plans for pregnancy, or difficulty taking the medicine regularly so the treatment plan can be reviewed.

Questions to raise

Ask what your results mean, why replacement is advised, how to take it with your other medicines, when monitoring is due, and which changes should trigger a dose review.

Questions people ask

Should I stop replacement when I feel better?

No. Feeling better does not show that the underlying hormone need has ended; discuss any change with your clinician.

Can the dose change later?

Yes. Results, symptoms, pregnancy and other medicines can affect the dose selected.
