When is subacromial decompression recommended for shoulder impingement?

Short answer
Subacromial decompression may be considered when shoulder pain and restricted function persist despite a thorough assessment and appropriate non-surgical care.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Decision basis
- Diagnosis, function, symptom persistence and response to non-surgical care.
- Procedure type
- Usually performed using a camera through small shoulder incisions.
Understanding the decision
Shoulder impingement describes pain linked to movement and loading around the space beneath the acromion. Similar symptoms can arise from rotator cuff problems, stiffness, arthritis or referred pain from the neck, so the diagnosis needs a clinical examination. A clinician may assess strength, range of motion and how symptoms respond to specific movements, with imaging when it adds useful information.
Decompression is an arthroscopic procedure intended to reshape or clear tissue around the subacromial space. It is not automatically the first choice for shoulder pain. The expected benefit, possible limits and alternatives should be discussed because symptoms may have more than one contributor.
What to do before deciding
Discuss a structured programme of activity modification, physiotherapy and pain management with your clinician. Give the plan enough time to show whether movement and strength are improving, while reporting worsening function or night pain. Ask whether the proposed procedure addresses the confirmed source of pain and what recovery involves. Further information is available on [Shoulder Impingement](/conditions/shoulder-impingement/) and [Subacromial Decompression](/treatments/sports-and-arthroscopic-surgery/subacromial-decompression/).
When to seek care promptly
Seek urgent assessment for a shoulder injury followed by inability to lift the arm, a visibly deformed joint, sudden major swelling, new numbness or a cold or discoloured hand. Fever, spreading redness or feeling acutely unwell with shoulder pain also needs prompt medical review.
Questions for your doctor
Ask what diagnosis the examination supports, which non-surgical options remain, and what improvement would justify continuing them. Discuss the purpose of decompression, likely restrictions, physiotherapy after surgery, and how another shoulder condition would be treated if it is found.

Questions people ask
Is decompression suitable for every painful shoulder?
No. Shoulder pain has several possible causes, and the procedure is considered only when assessment supports it.
Can physiotherapy help shoulder impingement?
A tailored programme may improve shoulder movement, strength and load tolerance, depending on the underlying problem.
