When is stoma surgery recommended for rectal cancer?

The rectum is the lower part of the large bowel, close to the anus. When a tumour is removed, the surgeon may join the bowel very low in the pelvis. A temporary ileostomy can divert stool away from this join while it heals. If the rectum and anal passage must be removed, a colostomy may provide the long-term route for stool. The decision also considers the tumour’s location, previous treatment, bowel health and whether the join can be made safely.

A temporary stoma is not an automatic promise of reversal. Reconnection may be considered after healing and follow-up assessment, and it can involve another operation. A specialist stoma nurse can prepare you for pouch care and help choose a comfortable site on the abdomen.

Before surgery, ask the colorectal surgeon to show you the planned operation and explain the possible stoma types. Request pre-operative stoma marking, ideally while standing and sitting, so the site can avoid folds, scars and the waistband. Learn how to empty and change a pouch, protect the surrounding skin and obtain supplies. If chemotherapy or radiotherapy is part of the plan, ask how treatment timing may affect surgery and recovery.

Call your care team if the stoma becomes dark, pale or very swollen, if output suddenly stops, or if watery output causes thirst, dizziness or reduced urination. New severe pain, repeated vomiting, fever or increasing abdominal swelling needs urgent assessment. Leaks and sore skin are common practical problems that still deserve early advice, because a fitting change can prevent further irritation.

Ask whether the stoma is likely to be temporary, what would be needed before reversal, how the operation may affect bowel control, and which symptoms should trigger a same-day call.
