When is scar revision surgery recommended for hypertrophic scar?

A hypertrophic scar is raised and firm but stays within the original wound boundary. It may feel itchy, tender, tight, or uncomfortable when clothing or movement rubs against it. Revision surgery reshapes or removes selected scar tissue and closes the area in a way intended to reduce tension. It cannot promise that the new scar will be flat, and some scars need additional treatment after healing.

Have the scar assessed for its age, activity, thickness, colour, mobility, and effect on movement. Tell the clinician about silicone sheets, pressure garments, steroid injections, laser care, or other measures already tried. Ask whether the scar is still changing and whether a less invasive option is suitable. Review the proposed incision, wound care, activity limits, sun protection, and the possibility of staged treatment if the scar crosses a joint or covers a broad area.

Arrange medical review if a raised scar grows beyond the original wound, becomes rapidly more painful, opens, bleeds, or produces discharge. Sudden warmth, spreading redness, swelling, or fever may indicate infection. Seek timely advice if tightness is reducing movement, affecting sleep, or interfering with work, because function may need attention while treatment options are being considered.

Ask how the scar differs from a keloid, what outcome is realistic, which non-surgical treatments could be tried, and how recurrence or renewed thickening would be monitored.
