When Is PUVA Therapy Recommended for Mycosis Fungoides?

Short answer
PUVA therapy may be recommended for selected skin-limited mycosis fungoides when patches are widespread, difficult to treat with creams, or have not responded adequately to other skin-directed care.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What this treatment means
PUVA combines a light-sensitising medicine with a carefully measured dose of ultraviolet A light. For mycosis fungoides, the aim is to reach abnormal skin cells in treated areas and reduce visible patches. A dermatologist or skin lymphoma team considers the stage, thickness, location and extent of the lesions before recommending it. PUVA is not automatically suitable for every person, and treatment planning also accounts for medicines, eye health, previous light exposure and the ability to attend repeated sessions.
What to do before starting
Bring a complete list of medicines and tell the team about eye conditions, liver problems, light sensitivity, previous skin cancers and any difficulty standing in a treatment booth. Ask how the sensitising medicine is taken, how your eyes and skin must be protected, and how response will be checked. Attend sessions on the schedule provided and report unusual burning, blistering, marked redness or new symptoms rather than changing the dose yourself.
When to seek care promptly
Contact the treating team promptly for severe redness, blistering, painful swelling, eye discomfort or visual changes after a session. Arrange an earlier review if lesions become thicker, ulcerated, rapidly more numerous, or associated with fever, persistent night sweating or unexplained weight loss. These changes need clinical assessment and should not be judged from photographs alone.
Questions for your doctor
Ask whether your mycosis fungoides is skin-limited, what treatment goal is realistic, how long a trial may last, and which alternatives would be considered if PUVA is unsuitable or ineffective.

Questions people ask
Is PUVA used for every type of mycosis fungoides?
No. Suitability depends on the disease stage, skin findings, medical history and whether the treatment can be delivered safely.
Can PUVA be used on its own?
It may be used as skin-directed treatment, but the specialist may combine or sequence it with other approaches depending on the findings.
