When is physiotherapy recommended for meniscus tear?

Short answer

Physiotherapy is recommended for a meniscus tear when the knee can be examined safely and symptoms allow gradual movement, strengthening and return to function.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How a meniscus tear can affect the knee

The menisci are cushioning pieces of cartilage between the thighbone and shinbone. A tear can lead to pain with turning, swelling, stiffness, clicking or difficulty squatting. Some tears are managed first with activity changes and rehabilitation; others need specialist review because the knee may catch, lock or remain unstable. An assessment considers the injury pattern, knee motion, strength and daily demands. Physiotherapy aims to improve how the knee is supported rather than forcing painful positions.

What therapy may include

A therapist may begin with comfortable range-of-motion exercises, thigh and hip strengthening, and advice on reducing movements that trigger catching or swelling. Progression can include step control, balance, cycling or task-specific practice. Complete the home exercises at the agreed frequency and report a delayed increase in swelling. Do not repeatedly test a locked or sharply painful knee. Treatment can continue after a procedure when a surgeon provides movement and loading limits.

When the knee needs prompt review

Seek prompt medical assessment if the knee locks and will not straighten, you cannot bear weight, swelling develops rapidly, or the joint repeatedly gives way. Urgent care is appropriate for severe pain with deformity, a cold or numb foot, or rapidly worsening symptoms. Persistent catching despite a rehabilitation plan also warrants review.

Questions for your clinician

Ask whether the tear appears suitable for rehabilitation, which activities should be paused, how progress will be measured, and when imaging or an orthopaedic opinion is needed. Ask which symptoms mean you should stop an exercise.

Questions people ask

Can a meniscus tear be treated with physiotherapy?

Rehabilitation may be appropriate for some tears, while locking, instability or ongoing symptoms may require further specialist assessment.

Should I exercise through knee pain?

Use only exercises that fit your plan; sharp pain, catching or increased swelling should be reported rather than pushed through.
