When is oral corticosteroid treatment recommended for erythema nodosum?

Erythema nodosum causes tender, warm lumps beneath the skin, commonly over the shins. It can occur after an infection, with inflammatory conditions, or after a medicine, so steroids are not automatically the first choice. A clinician may examine the lumps and ask about fever, sore throat, cough, bowel symptoms, joint pain, new medicines, and pregnancy. Treating the trigger may be more useful than suppressing inflammation alone.

Book an assessment if painful lumps are new, recurring, spreading, or interfering with walking. Resting the legs, elevating them, and using a clinician-approved pain reliever may help while the cause is assessed. Do not begin oral steroids without advice, especially if you have an untreated infection or diabetes. Record recent illnesses, medicines, travel, and associated symptoms to share at the visit.

Seek prompt care for severe fever, rapidly worsening pain, a hot expanding area, pus, an open sore, or feeling acutely unwell. Urgent assessment is also needed for breathing difficulty, chest pain, coughing blood, severe abdominal pain, or a painful swollen joint. These features may point to infection or a related illness requiring a different treatment approach.

Ask what possible trigger is being investigated, whether tests are needed before steroids, and how pain will be managed. Clarify the expected treatment course, tapering instructions, signs of infection, and when persistent or returning lumps should be reassessed.
