When is operative hysteroscopy recommended for uterine fibroids?

A hysteroscope is a slender camera passed through the vagina and cervix, so the inside of the uterus can be viewed without an abdominal incision. During an operative procedure, instruments can be passed through the hysteroscope to remove a suitable fibroid from the cavity. This approach is mainly considered for submucosal fibroids, which grow beneath the uterine lining and may bulge into the cavity. Fibroids located within the muscle wall or on the outer surface usually need a different discussion because hysteroscopy cannot reach them directly.

Your clinician may weigh the fibroid’s position and size, the amount and pattern of bleeding, anaemia symptoms, pregnancy plans, previous treatment, and findings from ultrasound or another scan. The recommendation is individual: a fibroid that causes no problems may not need removal, while symptoms that persist or interfere with daily life may support treatment.

Arrange a gynaecology assessment if you have ongoing heavy or irregular bleeding, pelvic pressure, or fertility concerns. Bring a record of bleeding days, flooding or clots, pain, fatigue, and any medicines. Ask whether imaging shows a cavity-distorting fibroid and whether hysteroscopic removal fits your goals. Before scheduling, discuss the setting, anaesthesia, expected recovery, alternatives, and how the plan changes if the fibroid cannot be removed in one procedure. If pregnancy is a goal, ask how treatment timing may affect attempts to conceive.

Seek urgent medical help for bleeding that rapidly soaks through pads, fainting, severe weakness, chest discomfort, or shortness of breath. Prompt assessment is also appropriate for sudden severe pelvic pain, fever with worsening pelvic pain, or bleeding after a procedure. These symptoms need assessment for blood loss, infection, or another urgent cause; they do not determine whether hysteroscopy is the right fibroid treatment.
