When is omalizumab therapy recommended for chronic spontaneous urticaria?

Chronic spontaneous urticaria causes recurring itchy wheals, with or without deeper swelling, without a consistently identified external trigger. Omalizumab is given by injection under medical supervision and works by changing an immune pathway involved in allergic-type inflammation. The clinician reviews how often hives occur, sleep disruption, swelling episodes, previous medicines, and other health conditions before recommending it. It is not a treatment for every rash that resembles hives.

Keep a symptom record showing when wheals or swelling appear and how long they last. Tell the specialist about asthma, allergies, pregnancy, other injections, and any previous reaction to medicines. Continue or change antihistamines only as directed. Attend monitoring visits and report new symptoms after an injection rather than taking an extra dose independently.

Get emergency help for swelling of the tongue or throat, trouble breathing, faintness, or rapidly worsening swallowing difficulty. Contact the treating team for widespread symptoms, severe injection-site reaction, or a new illness after treatment. Hives with bruising, pain, fever, or lesions that stay fixed need medical review because they may not be ordinary urticaria.
