What cachexia can change

Cancer cachexia involves changes in appetite, metabolism, muscle, and energy use; eating more alone may not reverse these changes. A dietitian can assess food and fluid intake, swallowing, symptoms, medicines, strength, and treatment aims. Support may include practical food changes, oral supplements, symptom control, or tube or intravenous nutrition in selected situations. The aim may be to maintain comfort, hydration, function, or strength for treatment, rather than to force eating when it causes distress.

What to do now

Tell the oncology team about reduced appetite, early fullness, taste changes, nausea, constipation, diarrhoea, pain, or trouble swallowing. Keep a simple record of what the person manages to eat and drink, and ask for a dietitian review early. Small frequent meals, nourishing drinks, and texture changes may be useful when advised for the person’s symptoms and treatment.

When to seek care promptly

Seek prompt medical advice if the person cannot keep fluids down, becomes confused or unusually drowsy, has very little urine, develops choking or breathing trouble while eating, or becomes too weak to stand safely. A sudden worsening of intake during chemotherapy or radiotherapy should be reported to the treatment team.
