When Is Neoadjuvant Chemotherapy Recommended for Inflammatory Breast Cancer?

Chemotherapy before surgery is generally recommended as an initial treatment for inflammatory breast cancer when the disease can be treated with an aim of controlling it locally and throughout the body. This approach is used because inflammatory breast cancer can involve a broad area of breast skin and lymphatic channels, making immediate surgery unsuitable in many cases.

Neoadjuvant chemotherapy is medicine given before an operation. The aim is to treat cancer cells that may be outside the breast, reduce activity in the breast and nearby lymph nodes, and assess how the tumour responds. The treatment plan may also include medicines selected according to hormone-receptor and HER2 test results.

If the cancer responds sufficiently and surgery is considered appropriate, treatment usually moves to breast surgery followed by radiotherapy and any recommended additional medicines. The exact sequence depends on scans, examination findings, biopsy results, overall health and the response during treatment.

Ask the breast cancer team how the diagnosis was confirmed, whether imaging has checked the breast and lymph nodes, and which tumour markers were found on the biopsy. These results help the team choose the medicines and decide whether targeted treatment or hormone treatment should be included.

Before treatment starts, tell the team about medicines, allergies, other health conditions, dental concerns and any possibility of pregnancy. Keep a record of fever, new symptoms and treatment dates. Do not delay a scheduled review if breast swelling, skin changes or pain are worsening.

Contact the oncology team promptly during chemotherapy for a fever or chills, sudden shortness of breath, chest pain, confusion, severe weakness, uncontrolled vomiting, or bleeding that does not stop. Treatment can lower the body's ability to handle infection, so the team may want to assess these symptoms quickly.

Seek urgent help for facial or throat swelling, wheezing or difficulty breathing, especially during or soon after an infusion. These can signal a serious reaction and need immediate assessment.
